How fees work
Patients here pay in four ways. This is the order most people ask in.
Insurance we work with
We send claims to dental plans for the procedures they cover, and to a medical plan when the visit is medical, such as a jaw injury or a biopsy. Whether we are in network with your plan is the first thing we tell you when you call. The plan decides what it pays; we tell you what to expect before surgery.

If you pay yourself
You get a written estimate for the visit, the procedure and any anesthesia before you decide. Pay at the visit, or ask the front desk what options there are. Financing is offered by third-party lenders. Approval and terms are set by the lender.

Receipts for reimbursement
After surgery you get an itemized statement with the procedure codes. Send it to a plan that reimburses you, or keep it for your records. Thomas Bolton can answer questions about it.

An estimate in advance
You can ask for a written estimate before any treatment. If you are paying without insurance, we give you a written estimate in advance, when you schedule or whenever you ask. It lists each step we expect and the fee for it.

Checking your benefits
Thomas Bolton checks your dental and medical benefits before your visit and calls you with what he found. Some plans want approval before surgery; he asks for it. You are welcome to call your plan too.

What the first visit includes
A consult with Dr. Applewhite, an exam, and an X-ray if you don't have a recent one from your dentist. You leave with a plan in writing and the estimate. Fill in your health history on our private patient portal, not by email or in the contact form.
